The IHS serves communities that face some of the country’s most severe healthcare access challenges, a reality that shaped nearly every technical decision the project required from the earliest planning stages.
Building for the Hardest Conditions
“We had to build something that performed under genuinely constrained conditions, not ‘rural US’ constrained, but settings where reliable connectivity could not be assumed at all,” Justin Fulcher said. He also pointed to the communities themselves, noting that they have “historically had reason to be cautious about new systems entering their healthcare environment,” which added a layer of trust building on top of the technical work already required to make the platform usable at all.
The platform used for the deployment incorporated artificial intelligence modules for clinical decision support, functionality that had to meet strict federal privacy and security standards before it could be used in practice. Fulcher said the discipline required to get that right mirrored what RingMD had already learned building for India and Indonesia, where connectivity gaps and regulatory scrutiny had already forced the team to design conservatively from the start.
Why the Work Mattered
“Engineer for the most demanding environment first, and everything else becomes more manageable,” he said, describing the operating principle behind the project. But Fulcher returns most often to the human outcome rather than the engineering challenge. “When a platform connects someone to a qualified doctor for the first time, the infrastructure question becomes secondary very quickly,” he said.
